Terms and Conditions
Last updated: July 1, 2026
Introduction
These Terms and Conditions (“Terms”) govern your use of the website located at https://reformationcounselingcenter.com (the “Site”) and the counseling and related services we provide (together with the Site, the “Services”). The Services are operated by Reformation Counseling Center LLC (“Reformation Counseling Center,” “we,” “us,” or “our”), a Colorado limited liability company with its principal place of business at 19590 E. Main St., Suite 200, Parker, CO 80138, United States.
You can reach us by:
- Phone: 720-915-2485
- Email: info@reformationcounselingcenter.com
- Mail: 19590 E. Main St., Suite 200, Parker, CO 80138, United States
- Office hours: Monday–Friday, 9:00 AM–5:00 PM
By using the Site, requesting a consultation, or scheduling an appointment with us, you (“you” or “User”) confirm that you have read these Terms, understand them, and agree to be legally bound by them. If you do not agree to any part of these Terms, please do not use the Site or request our Services.
We may update these Terms from time to time. The “Last updated” date above will reflect any revisions. Your continued use of the Site or Services after a change means you accept the revised Terms. The Site is intended for users who are at least 18 years of age.
These Terms govern the website and the business relationship. They do not replace the treatment agreement, informed consent, telehealth consent, financial agreement, and Colorado mandatory disclosure statement you sign when you begin counseling. Where those documents conflict with these Terms on a matter of your care, those documents control.

- How to Contact Us
1. About Our Services
Reformation Counseling Center provides mental health counseling to individuals, couples, families, adolescents, and children. Our services include individual counseling, marriage and couples counseling, family counseling, adolescent counseling, addiction counseling, play therapy, and parent coaching.
We see clients in person at our Parker office and by telehealth. Our counselors are licensed by the State of Colorado, and we can provide services only to clients who are physically located in Colorado at the time of the session. We serve Parker, Castle Rock, Lone Tree, Highlands Ranch, Littleton, Aurora, Denver, and surrounding communities.
The Site provides general information about our practice and our counselors, and includes forms to request a free, no-obligation consultation call.
We may decline to accept someone as a client — because of clinical fit, scope of practice, capacity, geography, or any other lawful reason. If we do, we will do our best to point you toward an appropriate referral.
2. No Therapist – Client Relationship Through the Site
Using this Site does not make you a client. Browsing the Site, reading our blog, submitting a form, exchanging emails or text messages with our office, or completing a free consultation call does not by itself create a counselor–client or therapist–client relationship.
That relationship begins only when you and a counselor at our practice have completed intake, you have received and signed the treatment agreement, informed consent, and Colorado mandatory disclosure statement, and the counselor has accepted you as a client.
Until then, nothing we say on the Site or in a preliminary conversation is clinical advice, a diagnosis, a treatment recommendation, or a promise of treatment. Please also read our Disclaimer at https://reformationcounselingcenter.com/disclaimer.
3. Not for Emergencies
The Site, our forms, our voicemail, our email, and our text line are not monitored for emergencies. They are monitored during business hours only — Monday through Friday, 9:00 AM to 5:00 PM. A message sent outside those hours may not be seen until the next business day.
If you or someone you know is in immediate danger, thinking about suicide, or experiencing a mental health emergency, get help now:
- 911 — for any immediate emergency
- 988 — call or text, available 24/7. This is both the national 988 Suicide & Crisis Lifeline and the 988 Colorado Mental Health Line.
- com — live chat with a Colorado crisis counselor, available 24/7
- The former Colorado Crisis Services numbers (1-844-493-8255 and text 38255) still work — as of July 1, 2025 they route to the 988 Colorado Mental Health Line.
- Your nearest emergency room or walk-in crisis center
4. Consultations, Scheduling, and Appointments
The free consultation
We offer a free consultation call of approximately fifteen (15) minutes. Its purpose is to understand what you are looking for, answer your questions, and determine whether we are a good fit. It is not a counseling session, not an assessment, and produces no diagnosis or treatment plan. There is no obligation on either side.
Requesting an appointment
Submitting a form or selecting a time on our scheduling page is a request, not a confirmed appointment. An appointment is confirmed only when you receive a confirmation from us. Availability shown online may change before we confirm.
Accuracy of your contact information
Please give us accurate contact information and tell us if it changes. We are not responsible for a missed appointment or an undelivered message caused by an incorrect phone number or email address, or by your carrier or email provider filtering our message.
Arriving at the office
Our office is at 19590 E. Main St., Suite 200, Parker, CO 80138. Please allow a few minutes to park and find the suite.
5. Fees and Payment
Session fees
Session fees are set by counselor and are disclosed to you before your first session and in your financial agreement. As of the “Last updated” date of these Terms, standard session fees range from $155 to $175 per session depending on the counselor. Fees are subject to change with reasonable notice.
Private pay – we do not bill insurance
Reformation Counseling Center is a private-pay practice. We do not bill insurance directly and we are not in network with any insurance plan. Payment is your responsibility at the time of service.
On request, we can provide a superbill and receipts suitable for submission to your insurer for possible out-of-network reimbursement, or for reimbursement through an FSA or HSA. We cannot guarantee that your insurer will reimburse any amount. Whether a claim is paid, and how much is paid, is between you and your insurer. If out-of-network reimbursement matters to you, call your insurer before your first session and ask what they cover for outpatient mental health services from an out-of-network provider.
Payment methods and timing
Payment is due at the time of service unless a different arrangement is agreed in writing. Credit card payments are handled by our payment processor; we do not store full card numbers.
Good Faith Estimate
Under the federal No Surprises Act, clients who are uninsured or who are not using insurance have the right to a Good Faith Estimate of the expected cost of services. Because we are a private-pay practice, this applies to essentially all of our clients. We provide a Good Faith Estimate in writing automatically when you schedule — within one (1) business day if your appointment is at least three (3) business days away, and within three (3) business days if it is at least ten (10) business days away — and within three (3) business days of any request. You may ask for one at any time, including before you schedule.
6. Cancellations and Missed Appointments
Please cancel or reschedule at least 24 hours before your appointment. Appointments cancelled with less notice, and appointments you do not attend, may be charged the full session fee, ranging from $155 – $175.
Because we are a private-pay practice, a missed-appointment fee is never covered by insurance and is your responsibility.
We understand that emergencies, illness, and Colorado weather happen, and we will use reasonable judgment. The specific policy that governs your care is the one in the financial agreement you sign.
We reserve the right to reschedule an appointment for weather, safety, illness, or staffing reasons. We will work with you to find an alternative time.
7. Telehealth
We offer counseling by secure video. If you receive telehealth services from us, the following applies in addition to the telehealth informed consent you will sign.
- Your location matters, not your residence. You must be physically located in the State of Colorado at the time of a telehealth session. Tell your counselor in advance if you will be traveling out of state so the session can be rescheduled rather than cancelled at the last minute.
- Telehealth sessions are conducted on TheraNest EHR, configured for HIPAA-compliant use under a business associate agreement.
- Your environment. You are responsible for a private space, a reliable internet connection, and a working device. We cannot control conditions on your end.
- Technology failures. Connections drop. Your counselor will attempt to reach you by phone if a session is interrupted. Establish a backup plan with them at the start of treatment.
- Not always appropriate. Telehealth is not clinically appropriate for every person or every concern. Your counselor may recommend in-person care or a higher level of care, and may decline to continue telehealth on clinical grounds.
- Because your counselor may be physically far away, you will be asked to provide your session location and an emergency contact so local emergency services can be directed to you if needed.
- Sessions are not recorded. Do not record a session without the express written consent of everyone present.
8. Your Treatment Documents and Records
Before your first session you will receive and sign a set of documents that govern your care: a treatment agreement, informed consent, a financial agreement, a telehealth consent if applicable, and the Colorado mandatory disclosure statement required by C.R.S. 12-245-216.
You will also receive our HIPAA Notice of Privacy Practices through our secure client records system when you onboard. That notice, not the Privacy Policy on this Site, describes how your protected health information may be used and disclosed and the rights you have over it. The Privacy Policy at https://reformationcounselingcenter.com/privacy governs the marketing website only.
Your clinical record is maintained in our secure client records system, not on this Site. Requests for access to your record should go to privacy@reformationcounselingcenter.com or be made directly to your counselor.
9. Communications With Us
When you contact us electronically or use the Site, you consent to receive communications from us electronically, and you agree that electronic notices, agreements, and disclosures satisfy any legal requirement that such communications be in writing.
Email and text message are not secure or confidential channels. They can be intercepted, misdirected, or read by anyone with access to your device. We use them for scheduling and administration. If you send us clinical information by email or text, you do so at your own risk and you consent to our receiving it that way. Ask us for a secure method if you need one.
Please keep web form submissions brief and general — your name, how to reach you, and in broad terms what you are looking for. Do not submit detailed symptom histories, medication lists, or records from another provider through the Site.
10. Site Use and Intellectual Property
We own or hold licenses to all content on the Site, including text, photographs, graphics, logos, designs, video, blog articles, and underlying software (the “Site Content”). The Reformation Counseling Center name and logo are our trademarks. All of this material is protected under United States and other applicable intellectual property laws.
We grant you a limited, revocable, non-transferable license to access and view the Site for the personal purpose of learning about our Services and contacting us. You may not copy, republish, scrape, repackage, or commercially exploit any Site Content without our prior written permission.
If you believe content on our Site infringes your copyright, contact us at info@reformationcounselingcenter.com with a description of the work, its location on our Site, and your contact information.
11. Prohibited Activities
You agree not to:
- Use the Site or our Services for any unlawful purpose or in violation of these Terms.
- Submit false, misleading, or fraudulent information, or impersonate another person.
- Submit another person’s contact information without their authority, or represent that you may authorize care for someone when you may not.
- Interfere with the operation of the Site, attempt to gain unauthorized access to any system, or use any automated tool, bot, scraper, or similar means to access the Site.
- Reverse engineer, decompile, or attempt to extract source code from any part of the Site.
- Harvest names, phone numbers, or email addresses from the Site.
- Use the Site to send unsolicited commercial messages.
- Post or transmit content that is harassing, defamatory, obscene, threatening, or otherwise unlawful, or that discloses another person’s private information.
- Use the Site in any manner that could disable, overburden, damage, or impair it.
We may suspend or terminate your access to the Site, or decline Services to any person, at our discretion for violation of these Terms or for any other lawful reason. Termination of Site access is separate from termination of a counseling relationship, which is governed by your treatment agreement and by professional and ethical standards, including reasonable notice and appropriate referral.
12. What You Send Us
By submitting content to us — a form, an email, a review, a comment, or feedback about the Site — you grant us a non-exclusive, royalty-free, worldwide license to use it for the purpose of responding to you and operating the practice. You represent that you have the right to send it and that it is not unlawful, defamatory, or infringing. We do not treat unsolicited business ideas or proposals as confidential.
This does not apply to information that becomes part of your clinical record, which is governed by HIPAA, Colorado law, and your treatment documents.
13. External Website Links
The Site may contain links to third-party websites — for example, a scheduling tool, a professional directory, a crisis resource, or a social media platform. We do not control these sites, do not necessarily endorse their content, and are not responsible for their accuracy, availability, or privacy practices. Visiting any linked site is at your own risk, and that site’s own terms and privacy policy apply.
14. Disclaimer
THE SITE AND THE SITE CONTENT ARE PROVIDED ON AN “AS-IS” AND “AS-AVAILABLE” BASIS. TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, IN CONNECTION WITH THE SITE, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED OR ERROR-FREE, THAT DEFECTS WILL BE CORRECTED, OR THAT THE SITE OR ITS SERVERS ARE FREE OF VIRUSES OR OTHER HARMFUL COMPONENTS.
CONTENT ON THE SITE, INCLUDING BLOG ARTICLES AND EDUCATIONAL RESOURCES, IS GENERAL INFORMATION FOR A GENERAL AUDIENCE. IT IS NOT CLINICAL ADVICE, NOT A DIAGNOSIS, AND NOT A SUBSTITUTE FOR CONSULTATION WITH A QUALIFIED PROFESSIONAL WHO KNOWS YOUR SITUATION. WE DO NOT GUARANTEE ANY PARTICULAR OUTCOME OR RESULT FROM COUNSELING.
Our full Disclaimer, at https://reformationcounselingcenter.com/disclaimer, is incorporated into these Terms by reference. Nothing in these Terms limits our professional obligations to you as a client, or any liability that cannot be limited or excluded under Colorado or federal law.
15. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, IN NO EVENT WILL REFORMATION COUNSELING CENTER LLC OR ITS OWNERS, MEMBERS, EMPLOYEES, OR CONTRACTORS BE LIABLE TO YOU FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, OR PUNITIVE DAMAGES, INCLUDING LOST PROFITS, LOST GOODWILL, LOSS OF USE, OR LOSS OF DATA, ARISING OUT OF OR RELATED TO YOUR USE OF THE SITE, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR TOTAL AGGREGATE LIABILITY TO YOU FOR ANY CLAIM ARISING OUT OF OR RELATED TO THE SITE WILL NOT EXCEED THE GREATER OF ONE HUNDRED DOLLARS ($100) OR THE AMOUNT YOU PAID US FOR SERVICES IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM. SOME STATES DO NOT ALLOW THE LIMITATION OR EXCLUSION OF CERTAIN DAMAGES, SO PARTS OF THIS SECTION MAY NOT APPLY TO YOU, AND YOU MAY HAVE ADDITIONAL RIGHTS UNDER STATE LAW.
This section limits liability arising from the website. It does not apply to, and we do not attempt to limit, liability arising from the professional counseling services we provide, from our own gross negligence or willful misconduct, or any liability that cannot lawfully be limited.
16. Indemnification
You agree to defend, indemnify, and hold harmless Reformation Counseling Center LLC and its owners, members, employees, and contractors from and against any claim, demand, loss, liability, damage, or expense (including reasonable attorneys’ fees) arising out of or related to: (a) your use of the Site; (b) your breach of these Terms; (c) your violation of any third party’s rights; or (d) any inaccurate information you provided to us. This does not apply to claims arising from our own professional services, negligence, or misconduct.
17. Governing Law
These Terms and your use of the Site and Services are governed by the laws of the State of Colorado, without regard to its conflict-of-laws rules.
18. Dispute Resolution
Informal resolution first
If you have a concern, please tell us first. Most problems are solved by a conversation. Contact us at 720-915-2485 or info@reformationcounselingcenter.com and we will make a good-faith effort to resolve the issue within thirty (30) days.
Concerns about professional conduct
If your concern is about the professional conduct of one of our counselors, you may contact the Colorado Department of Regulatory Agencies, State Board of Licensed Professional Counselor Examiners, 1560 Broadway, Suite 1350, Denver, CO 80202, 303-894-7800. Contacting the Board is your right, and we will not retaliate against you for it. Nothing in these Terms limits your ability to file a complaint with any regulatory body.
Venue
If a dispute cannot be resolved informally, any legal action must be filed in the state courts located in Douglas County, Colorado, or in the United States District Court for the District of Colorado. You consent to personal jurisdiction in those courts.
19. General Provisions
These Terms, together with our Privacy Policy and Disclaimer, constitute the entire agreement between you and us regarding the Site. They do not supersede your signed treatment documents. Our failure to enforce any provision is not a waiver of that provision. If any provision is found unenforceable, it will be modified to the minimum extent necessary and the remaining provisions will remain in full effect. You may not assign these Terms; we may assign them in connection with a merger, acquisition, or sale of the practice. Neither party is a partner, joint venturer, agent, or employee of the other. Section headings are for convenience only. Sections that by their nature should survive termination — intellectual property, disclaimers, limitation of liability, indemnification, and governing law — will survive.
Accessibility. If you encounter a barrier to accessing any part of the Site, or need information in another format, contact us at info@reformationcounselingcenter.com or 720-915-2485 and we will work with you to provide what you need.
20. SMS Terms & Conditions
Program description
Reformation Counseling Center LLC operates an SMS messaging program to communicate with clients and prospective clients. Messages may include appointment reminders, scheduling and consultation confirmations, intake follow-ups, and practice notifications, and — if you separately opt in to marketing — occasional informational and promotional messages about our services, groups, and mental health resources.
How to join
You can join by checking the SMS consent box on a form at https://reformationcounselingcenter.com/contact or on our scheduling pages, by texting START to 720-915-2485, or by signing a written consent at our office. You may also give consent verbally during a call with our office for appointment and scheduling messages only — promotional messages require written or electronic consent.
Consent to receive text messages is not a condition of receiving counseling services or of any purchase. You can decline and still become a client.
Message frequency
Message frequency varies. Expect approximately 2 – 6 messages per month, depending on how often you have appointments.
Message and data rates
Message and data rates may apply. Reformation Counseling Center LLC does not charge you for these messages; your mobile carrier’s standard messaging and data rates apply. If you have questions about your text plan or data plan, contact your wireless provider.
Opting out
You can cancel the SMS service at any time. Just text STOP to 720-915-2485, or reply STOP to any message you receive from us. After you send STOP to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. We also honor UNSUBSCRIBE, CANCEL, END, and QUIT. You may also opt out by calling our office or emailing privacy@reformationcounselingcenter.com. Opting out will not affect your care — we will simply reach you by phone or email instead.
Rejoining
If you want to join again, just sign up as you did the first time, or text START to 720-915-2485, and we will start sending SMS messages to you again.
Support
If you are experiencing issues with the messaging program, reply with the keyword HELP for more assistance, or get help directly at 720-915-2485 or info@reformationcounselingcenter.com.
Carrier liability
Carriers are not liable for delayed or undelivered messages. Message delivery is subject to transmission limitations, carrier availability, and factors outside our control. Never rely on a text message for anything urgent or time-critical.
Supported carriers
Our program is supported by major U.S. carriers, including AT&T, Verizon Wireless, T-Mobile, and others. Not all carriers are covered, and carrier support may change without notice.
Changing your number
If you change or give up your mobile number, please tell us promptly so that messages intended for you do not reach someone else.
Confidentiality
Text messaging is not a secure or confidential channel. We limit our texts to scheduling and logistics and ask that you do the same. Do not send clinical details by text, and never use text messaging for a crisis — see Section 3.
Privacy
If you have any questions regarding privacy, please read our Privacy Policy at https://reformationcounselingcenter.com/privacy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
