Disclaimer
Last updated: July 1, 2026
This Disclaimer explains what the website at https://reformationcounselingcenter.com (the "Site") is, what it is not, and the limits of everything you read here. It applies alongside our Terms and Conditions and our Privacy Policy. The Site is operated by Reformation Counseling Center LLC ("Reformation Counseling Center," "we," "us," or "our"), 19590 E. Main St., Suite 200, Parker, CO 80138.
If anything here is unclear, please ask. We would rather explain it than have you guess. Call us at 720-915-2485 or email info@reformationcounselingcenter.com.
If You Are in a Crisis
This Site is not monitored for emergencies. If you or someone you know is in immediate danger, thinking about suicide, or experiencing a mental health emergency, do not use this Site, our contact form, email, or text messaging. Get help now:
- 911 — for any immediate emergency
- 988 — call or text, available 24/7. This is both the national 988 Suicide & Crisis Lifeline and the 988 Colorado Mental Health Line.
- 988colorado.com — live chat with a Colorado crisis counselor, available 24/7
- The former Colorado Crisis Services numbers (1-844-493-8255 and text 38255) still work — as of July 1, 2025 they route to the 988 Colorado Mental Health Line.
- 1-800-799-7233 — National Domestic Violence Hotline
- Your nearest emergency room or walk-in crisis center
Our office, forms, voicemail, email, and text line are monitored during business hours only — Monday through Friday, 9:00 AM to 5:00 PM. A message left outside those hours may not be seen until the next business day. Never assume a message has been received.

- How to Contact Us
1. General Information Only
Reformation Counseling Center publishes this Site to describe our practice and to share general information about mental health. Everything on it — service pages, counselor profiles, blog articles, downloadable resources, coping exercises, social media posts, and any videos — is general educational content intended for a general audience.
It was not written about you. It does not account for your history, your circumstances, your diagnosis, your medications, or anything else that would matter if we were actually working with you.
2. Not Medical or Mental Health Advice
Nothing on this Site is intended to be, and nothing on it should be treated as:
- A diagnosis or an assessment of any condition;
- A treatment plan or a recommendation for treatment;
- Medical advice, or advice about starting, stopping, or changing any medication;
- A substitute for consultation with a qualified licensed professional who knows your situation;
- Legal, financial, or other professional advice of any kind.
Talk to a licensed mental health professional, your physician, or another qualified provider about anything that concerns you. Never disregard professional advice, and never delay seeking it, because of something you read here. Never start or stop a prescribed medication based on website content — talk to the prescriber.
3. No Therapist–Client Relationship
Reading this Site, downloading a resource, following us on social media, submitting a contact or scheduling form, emailing or texting our office, or completing a free fifteen-minute consultation call does not create a counselor–client or therapist–client relationship.
That relationship begins only after you and a counselor at our practice have completed intake, you have received and signed the treatment agreement, informed consent, and Colorado mandatory disclosure statement, and the counselor has accepted you as a client. Until that happens, no counselor here owes you the professional duties that come with treatment, and nothing said to you is clinical advice.
The free consultation call exists to see whether we are a good fit. It is not therapy, not an assessment, and produces no diagnosis.
4. Licensure and Where We Can Practice
Our counselors are licensed by the State of Colorado. We can provide counseling services only to clients who are physically located in Colorado at the time of the session, whether in person or by telehealth. This is a legal limit on where a Colorado license reaches, not a preference.
If you live elsewhere, or if you are traveling out of state, we cannot see you for that session. Please tell your counselor in advance about travel so the session can be rescheduled. If you are outside Colorado and looking for care, we are glad to point you toward a licensing board directory in your state.
Nothing on this Site is an offer to practice counseling in any jurisdiction where our counselors are not licensed.
5. Clinical Supervision
Colorado licenses counselors at different levels. Some of our clinicians hold a full Licensed Professional Counselor (LPC) license. Others hold a Licensed Professional Counselor Candidate (LPCC) credential and practice under the clinical supervision of a fully licensed counselor while accumulating the post-graduate hours required for full licensure.
Supervision means a supervising counselor reviews case material and provides clinical oversight. That is a limit on confidentiality you should know about before you begin, and your counselor will explain it during informed consent.
| Counselor | Credential and license number | Supervision |
|---|---|---|
| Michael Everson | LPC, LAC | Independently licensed |
| Shannon Pannell | MA, LPC, NCC — LPC.0022790 | Independently licensed |
| Crystal Galloway | LPCC — LPCC.0023953 | Supervised by Michael Everson, LPC |
| Mike Nicastro | LPCC — LPCC.0023238; DVOMB Associate Level Provider | Supervised by Michael Everson, LPC |
You may verify any counselor's license status at any time through the Colorado Department of Regulatory Agencies at https://dpo.colorado.gov/ProfessionalCounselor.
6. Colorado Mandatory Disclosure Statement
Colorado law requires every psychotherapist to give each client a written disclosure statement before providing services. It covers the therapist's degrees, credentials, and experience; the fee structure; your right to receive information about the methods of therapy and the techniques used; your right to seek a second opinion; your right to terminate therapy at any time; the fact that sexual contact between a therapist and a client is prohibited and should be reported; and the limits of confidentiality.
This website is not that document and does not substitute for it. You will receive your counselor's mandatory disclosure statement in writing before your first session and will have the opportunity to ask questions about it.
Colorado's regulatory body for psychotherapists is:
State Board of Licensed Professional Counselor Examiners
Colorado Department of Regulatory Agencies, Division of Professions and Occupations
1560 Broadway, Suite 1350
Denver, CO 80202
Phone: 303-894-7800
Website: https://dpo.colorado.gov/ProfessionalCounselor
You have the right to contact the Board with a concern about a counselor's professional conduct, and doing so will not be held against you.
7. No Guarantee of Results
Counseling is not a product with a predictable output. Outcomes depend on many things — what you are working through, your history, your engagement, your circumstances outside the room, and factors nobody can control.
We do not guarantee any particular outcome, improvement, or result from counseling or from anything on this Site. Any statement about what counseling can do describes what is possible, not what is promised. Another person's experience does not predict yours.
Counseling can also be uncomfortable. Working through difficult material may temporarily bring up strong feelings or make things feel harder before they feel better. Your counselor will discuss the benefits and the risks with you as part of informed consent.
8. Telehealth Limitations
We offer counseling by secure video. Telehealth has real limitations you should understand:
- It is not clinically appropriate for everyone or for every concern. Your counselor may recommend in-person care or a higher level of care.
- A counselor on video cannot observe everything they would observe in a room.
- Technology fails. Connections drop, audio cuts out, and platforms have outages.
- In an emergency, your counselor may be physically far from you and dependent on emergency services in your area.
- Privacy on your end is your responsibility — the room you are in, the device you use, and who can overhear you.
- You must be physically located in Colorado at the time of the session.
You will review and sign a separate telehealth informed consent before beginning telehealth services.
9. Confidentiality of Web Forms, Email, and Text
Communications sent through this Site, by email, or by text message are not secure and not confidential. They can be intercepted, misdirected, stored on servers we do not control, or seen by anyone with access to your device.
Please keep these channels to scheduling and logistics. Do not send detailed symptom histories, medication lists, records from another provider, or anything you would not want read by someone else. Once you are a client, your counselor will tell you how to send sensitive information securely through our client records system.
Information you submit before becoming a client is not part of a clinical record and is not protected health information under HIPAA. We treat it as confidential and handle it as described in our Privacy Policy at https://reformationcounselingcenter.com/privacy, which explains how it is used, how long we keep it, and the website analytics and advertising tools that operate on our public pages.
10. Testimonials and Reviews
Professional ethics and Colorado regulation place strict limits on soliciting testimonials from current clients or from anyone vulnerable to undue influence. We do not solicit testimonials from current clients.
Where any client experience appears on this Site, it is used with written permission, is not paid for, and is edited only for length and to remove identifying details. Individual experiences are not representative and do not predict anyone else's results.
Reviews posted by third parties on Google, Facebook, Psychology Today, or similar platforms are written by those individuals. We do not control them, do not verify them, and do not adopt them as our own statements. We also do not confirm or deny whether any reviewer is or was a client — doing so would itself disclose protected information.
11. Blog, Articles, and Downloadable Resources
Our blog and any worksheets, guides, or exercises we offer are general educational material. They are accurate to the best of our knowledge as of the date published. Mental health research changes, and we do not commit to updating older content.
A coping technique described here — a grounding exercise, a breathing practice, a journaling prompt — is offered as general information. It may not be right for you, and it is not a treatment plan. If a technique makes you feel worse, stop and talk to a professional.
Views expressed in an article are those of its author and do not necessarily represent the position of every counselor at the practice.
12. External Links and Third-Party Content
We link to outside websites, hotlines, directories, and resources as a convenience. Those links do not constitute an endorsement of everything on the linked site, and we have no control over their content, accuracy, availability, or privacy practices. Once you leave our Site, our policies no longer apply. Use your own judgment.
13. Automated Tools and Chat
Parts of our Site, scheduling, and messaging may use automated tools — chat widgets, automated replies, appointment reminders, and scheduling assistants. These are administrative tools, not counselors. They do not provide clinical advice, cannot assess your situation, are not monitored continuously, and must never be used in an emergency. When you need a person, call 720-915-2485.
14. Advertising
We advertise our practice on search engines, social media platforms, and professional directories. Some listings where you may find us are paid placements. Being listed in a paid directory is not a third-party endorsement or a rating of quality. We do not accept payment from any third party to feature or recommend their products or services on this Site.
15. Accuracy and Availability
We work to keep this Site accurate and current, but we make no warranty that it is complete, current, or error-free. Fees, availability, hours, service offerings, and staff can change without the website catching up immediately. Confirm anything you plan to rely on by calling us at 720-915-2485.
The Site is provided "as is" and "as available." We do not warrant uninterrupted or error-free operation, and we may modify or discontinue any part of it at any time.
16. Limitation of Liability
To the fullest extent permitted by law, Reformation Counseling Center LLC and its owners, members, employees, contractors, and agents are not liable for any loss or damage arising from your use of, or reliance on, this Site or its content — including any decision you make, or action you take or fail to take, based on it.
This does not limit our professional responsibility to our clients, our liability for our own gross negligence or willful misconduct, or any liability that cannot lawfully be limited. The full terms are in our Terms and Conditions at https://reformationcounselingcenter.com/terms-and-conditions.
17. Updates to This Disclaimer
We may update this Disclaimer from time to time. The "Last updated" date at the top will reflect any changes. Continuing to use the Site after a change means you accept the update.
